Quick answer: Foundayo (orforglipron), Eli Lilly's daily GLP-1 pill for weight management, costs $149 a month at the 0.8 mg starting dose through LillyDirect self-pay and $299 a month at 9 mg and above. Prices checked Oct 13, 2026.

  • By dose: $149 at 0.8 and 2.5 mg, $199 at 5.5 mg, $299 at 9, 14.5 and 17.2 mg. The 2.5 and 5.5 mg prices are reduced through Dec 31, 2026.
  • With commercial insurance that covers it: as little as $25 a month with Lilly's savings card (up to $100 off a month, $1,000 a year; ends Dec 31, 2026).
  • Medicare: $50 a month for eligible Part D members through the Medicare GLP-1 Bridge (July 2026 to Dec 2027). Medicaid coverage depends on your state.
  • First year: about $3,188 stepping up to 9 mg, or $2,288 staying at 5.5 mg, at today's prices (our arithmetic). A telehealth membership adds $700 to $2,000 a year.

Foundayo price by dose

Lilly prices Foundayo by dose, and the label raises the dose in steps: 0.8 mg for at least 30 days, then 2.5 mg for at least 30 days, then 5.5 mg. After that a prescriber may move up to 9, 14.5 or 17.2 mg, a month or more apart, depending on response and tolerability (Foundayo prescribing information).

LillyDirect self-pay, one month (30 tablets), from LillyDirect and Lilly's savings terms. Checked Oct 13, 2026.
DosePrice nowRegular priceCondition
0.8 mg$149$149Starting dose (at least 30 days)
2.5 mg$149$199Reduced price through Dec 31, 2026 (at least 30 days)
5.5 mg$199$299Reduced price through Dec 31, 2026

14.5 mg$299$349$299 if refilled within 45 days
17.2 mg$299$349Maximum dose; $299 if refilled within 45 days

So the starting price is not the price you will pay for long. By month three you are on at least 5.5 mg, which costs $199 now and $299 at the regular price.

Where to get Foundayo, and what it costs all-in

What this covers: LillyDirect, which sells Foundayo directly, and the telehealth programs we could confirm list Foundayo on their own site, 6 options in all. It is not every provider on the market. Every option here is the same FDA-approved drug at Lilly's price; what differs is the membership or program fee on top. How it is sorted: by the lowest all-in monthly price at the 0.8 mg starting dose, on any plan length, counting every fee the seller requires (membership or program + medication)..

Sorted by the lowest all-in monthly price at the 0.8 mg starting dose, on any plan length, counting every fee the seller requires (membership or program + medication). Ties share a rank. Prices checked Oct 13, 2026 on each seller's own site unless the row says otherwise.

*First-year estimate is our arithmetic, not a quote: one 30-day fill at 0.8, 2.5 and 5.5 mg, then 9 months at 9 mg, at today's LillyDirect prices ($3,188 for the medication), plus the membership on the plan in the monthly column. LillyDirect's row excludes the prescriber, which you arrange yourself. Ro's figure was last confirmed on our Ro review on Oct 10, 2026. Hims & Hers said in April 2026 that its clinicians can send Foundayo prescriptions to LillyDirect, but we could not confirm a current Foundayo price on their pages, so they are not in the table. A retail pharmacy can also fill Foundayo; its cash price may differ, so ask before you fill. The seller's checkout is the final word on price.

Foundayo first-year cost

Using LillyDirect's prices and the label's minimum step-up schedule (our arithmetic, 12 monthly fills):

  • Stepping up to 9 mg (or 14.5 or 17.2 mg, refilled within 45 days): $3,188 at today's prices; $3,338 at Lilly's regular prices.
  • Staying at 5.5 mg: $2,288 at today's prices; $3,338 at regular prices.

The reduced 2.5 and 5.5 mg prices end Dec 31, 2026, so a year that starts now will mostly be billed at regular prices. Your prescriber decides the dose; these are budgets, not recommendations.

Foundayo with insurance: savings card and coverage

  • Covered by your commercial plan: Lilly's Foundayo Savings Card can bring a 1-, 2- or 3-month fill to as little as $25, with savings capped at $100 per month and $1,000 per calendar year, up to 10 fills a year. It ends Dec 31, 2026.
  • Commercial plan that does not cover it: the same card gives the self-pay prices above ($149 to $299 a month).
  • Coverage: plan by plan, usually with prior authorization. CVS Caremark removed its new-to-market block on Foundayo effective June 1, 2026 (Managed Healthcare Executive, May 28, 2026); whether your plan includes it is your employer's or insurer's call. See how to get a GLP-1 covered by insurance.
  • FSA and HSA: Lilly lists Foundayo self-pay as FSA/HSA eligible, varying by plan; prescribed medication can be a medical expense under IRS Publication 502. More in Can You Use HSA/FSA for a GLP-1?

Does Medicare or Medicaid cover Foundayo?

Medicare: eligible Part D members can get Foundayo for weight management for $50 a month through the Medicare GLP-1 Bridge, July 1, 2026 to Dec 31, 2027. It needs a prior authorization and BMI-based clinical criteria (35 or higher, or lower with certain weight-related conditions; see LillyDirect's Bridge page). Medicaid: coverage of weight-loss drugs is a state decision, so check your state plan. Lilly's savings card excludes Medicare, Medicaid and other government programs.

Foundayo vs Wegovy pill vs Zepbound: self-pay cost

At the starting dose, the two GLP-1 pills cost the same and the injection costs about twice as much. Foundayo is a different molecule from semaglutide and tirzepatide, not a pill version of either.

Self-pay prices from each maker, checked Oct 13, 2026: LillyDirect Foundayo, Wegovy self-pay (Novo Nordisk), LillyDirect Zepbound. Medication only.

*Our arithmetic at today's prices, not quotes: Foundayo stepping to 9 mg (above); Wegovy pill 1 month at 1.5 mg, 1 at 4 mg, then 10 at 9 mg; Zepbound 1 month at 2.5 mg, 1 at 5 mg, then 10 at 7.5 mg. Different drugs with different trial results; your prescriber chooses the drug and dose.

For the trial results side by side, see Foundayo vs Zepbound and tirzepatide vs semaglutide. Zepbound's prices by dose and every way to buy it are in our tirzepatide price table; the Wegovy pill's fasting rules are compared in oral vs injectable semaglutide. Wegovy and Ozempic prices at every dose, next to compounded semaglutide, are in our semaglutide price list.

Is there a generic or compounded Foundayo?

No generic exists: Foundayo, FDA-approved on April 1, 2026, is available only as the brand from Eli Lilly. A product sold online as "orforglipron" outside a prescription filled by a licensed pharmacy is not Foundayo. If you would consider a weekly injection instead, telehealth providers sell compounded semaglutide and tirzepatide; these are not FDA-approved and are not the same as Wegovy or Zepbound (FDA's compounding overview). Our cheapest GLP-1 cost comparison lists those prices next to the brand-name options.

Frequently asked questions

How much does Foundayo cost without insurance?

Through LillyDirect self-pay, Foundayo costs $149 a month at the 0.8 mg starting dose and $299 a month at 9 mg and above, as of Oct 13, 2026. A prescription is required, and telehealth programs add their own membership fee.

Does Foundayo cost more at higher doses?

Yes: $149 at 0.8 and 2.5 mg, $199 at 5.5 mg and $299 at 9, 14.5 and 17.2 mg. The 2.5 and 5.5 mg prices are reduced only through Dec 31, 2026, after which Lilly lists them at $199 and $299.

Is there a Foundayo savings card or coupon?

Yes: with commercial insurance that covers Foundayo, Lilly's savings card can bring a fill to as little as $25, with savings capped at $100 a month and $1,000 a year. The card excludes Medicare, Medicaid and other government plans and ends Dec 31, 2026.

Does insurance cover Foundayo?

Some commercial plans do, often with prior authorization; coverage depends on your employer or insurer, not just the pharmacy benefit manager. CVS Caremark lifted its new-to-market block on Foundayo on June 1, 2026.

Does Medicare cover Foundayo?

Eligible Medicare Part D members can get Foundayo for $50 a month through the Medicare GLP-1 Bridge, which runs from July 1, 2026 to Dec 31, 2027 and requires prior authorization. Lilly's $25 savings card does not apply to Medicare.

How much does Foundayo cost per year?

About $3,188 in the first year at today's LillyDirect prices if the dose steps up to 9 mg, or $2,288 if it stays at 5.5 mg; at Lilly's regular prices both come to $3,338. Telehealth membership fees are extra.

Is Foundayo cheaper than the Wegovy pill?

Both start at $149 a month and reach $299 at higher doses, so self-pay costs are close. Foundayo is $149 at its second step (2.5 mg) through Dec 31, 2026, while the Wegovy pill is $199 at 4 mg.

Is Foundayo cheaper than Zepbound?

Paying cash, yes: Foundayo runs $149 to $299 a month versus $299 to $449 for Zepbound through LillyDirect. They are different drugs with different trial results.

Is there a generic Foundayo?

No. Foundayo (orforglipron) is available only as the FDA-approved brand from Eli Lilly.

Can you use HSA or FSA money for Foundayo?

Lilly lists Foundayo self-pay as FSA/HSA eligible, though eligibility can vary by plan. Check with your plan administrator.
